Tips for High-Quality Hand-Offs Released with New TJC Sentinel Event Alert
While all healthcare professionals typically take great pride in the work they do to provide the best possible care for their patients, their efforts are too often sabotaged because communication falls short when patients are handed off or transitioned from one team member to another. To reduce opportunity for potential harm during transitions of care, The Joint Commission has released a new Sentinel Event Alert aimed at addressing inadequate communication between care givers during patient hand-offs.
Sentinel Event Alert, Issue 58 offers advice to both senders and receivers of information at the time of a hand-off and provides direction for caregivers within the hospital as well as those in care settings outside of the hospital.
In addition to the alert, TJC released an infographic that details 8 tips that can assist all caregivers in accomplishing high-quality hand-offs.
