11 Point Survey Assesses Patient Safety in Pharmacies
A toolkit developed and distributed by the Agency for Healthcare Research and Quality is making it easy for community pharmacies to assess patient safety in their environments and to discover areas where improvement is needed. The central component of the toolkit is the Pharmacy Survey on Patient Safety Culture, which allows pharmacy clerks, technicians and pharmacists to measures 11 areas of patient safety culture including physical space and environment, patient counseling, communication across shifts and teamwork.
The comprehensive survey also inquires about the frequency of documenting mistakes, and it delivers an overall rating on patient safety for the facility. Materials included in the toolkit include the survey form, a document identifying items by composite and a Survey User’s Guide.
